Third stage of labor 
Average 15 min, reduced to 5 minutes with active management of labor 
Active management- less PPH
-OXYTOCIN -10-40U/L in infusion… IV bolus can cause tachycardia and hypotension
-CARBOPROST – Max 8 doses (2gm) 15-90 minute apart avoid in asthma and hypertension 
MISOPROST 600mg for active management-sublingual, vaginal, buccaneers, oral … low cost easy to store and administer by untrained staff
-CCT
-Fundal massage 

Cause of PPH – 4 Ts
  • Trauma 
  • Tone
  • Tissue – retained placenta or accreta 
  • Thrombosis – coagulation defects congenital or acquired
MRP
  • Repair tears vaginal and cervical
  • Medical management- oxytocin, misoprost, carboprost 
  • Uterine packing
  • Uterine ballon tamponade
  • Selective devascularisation
  • BLynch suture
  • Internal iliac ligation
  • Hysterectomy
